Two ways to register. Pick whichever you prefer.
You can fill out your new-patient paperwork online before you arrive — it takes about five minutes — or you can fill out paper forms in the office on the day of your visit. Both work. Both get you the same care.
Online registration
The faster route — about five minutes.
The online registration form covers everything we need before your first visit: contact information, medical history, medications, insurance details if you have coverage, and your signed consent forms. It opens in a secure portal and your information is encrypted on the way to our records system.
Completing it before you arrive means you can walk in, check in at the desk, and be seated for your appointment without spending the first ten minutes on a clipboard.

Prefer to fill out forms in the office? That works too.
If you would rather skip the online portal, just plan to arrive about fifteen minutes early. The front desk will hand you a clipboard with the same information — contact, medical history, medications, insurance — and you can fill it out in the waiting room.
Bring a photo ID, your dental insurance card if you have one, and a list of medications and supplements you take.
